Strategies to consider: telestroke and 24-hour primary stroke centres.

چکیده
The stroke team in Hong Kong only performs duty during office hours due to limited resources,1 but 57% of strokes occur during non-office hours (from Queen Elizabeth Hospital [QEH]; data unpublished). Queen Elizabeth Hospital, the only hospital with 24-hour stroke service, treats roughly 1800 patients a year (QEH; data unpublished). Since there are approximately 18 000 stroke patients annually, this amounts to a whooping 9234 or so of stroke patients unable to receive immediate care. In developing countries like Pakistan, 25% of stroke patients arriving at one hospital within the recommended 3-hour time frame2 were eligible for recombinant tissue plasminogen activator (rtPA) treatment, but as of 2011, only 10% of Hong Kong stroke patients were able to achieve that (QEH; data unpublished). This rate has not been improved since 1999,3 rendering new measures necessary.

REACH: clinical feasibility of a rural telestroke network.
BACKGROUND AND PURPOSE Development of stroke networks is critical to bringing guideline-driven stroke care to rural, underserved areas. METHODS A Web-based telestroke tool, REACH, was developed to provide a foundation for a rural stroke network that delivered acute stroke consults 24 hours per day 7 days per week to 8 rural community hospitals in Georgia. RESULTS There were 194 acute stroke...
متن کاملThe TeleStroke Mimic (TM)‐Score: A Prediction Rule for Identifying Stroke Mimics Evaluated in a Telestroke Network
BACKGROUND Up to 30% of acute stroke evaluations are deemed stroke mimics (SM). As telestroke consultation expands across the world, increasing numbers of SM patients are likely being evaluated via Telestroke. We developed a model to prospectively identify ischemic SMs during Telestroke evaluation. METHODS AND RESULTS We analyzed 829 consecutive patients from January 2004 to April 2013 in our...
متن کاملTelestroke: An Approach to the Shortage of Neurologists in Rural Areas
Telestroke has provided swift, lifesaving treatment and has afforded patients the ability to be treated in an efficient manner in rural areas where neurologists may not always be available. It has been reported that Telestroke in rural areas has increased the ability to treat patients by being able to access a neurologist for assistance in evaluating the patient to determine if administering li...
متن کاملAssimilation of Web-Based Urgent Stroke Evaluation: A Qualitative Study of Two Networks
BACKGROUND Stroke is a leading cause of death and serious, long-term disability across the world. Urgent stroke care treatment is time-sensitive and requires a stroke-trained neurologist for clinical diagnosis. Rural areas, where neurologists and stroke specialists are lacking, have a high incidence of stroke-related death and disability. By virtually connecting emergency department physicians ...
متن کاملA Framework for Telestroke Network Design
Stroke is the third leading cause of death and leading cause of adult long-term disability in western industrialized countries. In stroke units providing special expertise, stroke patients receive highly efficacious care (Pollack et al., 2007). However, stroke units cannot be implemented in particular in sparsely populated rural areas because of a shortage of experienced neurologists (Audebert ...
